Metal Slug XX est un jeu d'action. Choisissez vos personnages parmi Marco, Tarma, Eri, Fio, Ralf et Clark (ces deux derniers étant issus de la série King of Fighters) et lancez-vous à l'assaut des sept niveaux bourrés d'ennemis et comprenant différents embranchements. Le jeu propose un mode 2 joueurs et un mode "Combat School" proposant plus de soixante dix missions.
